I work at a company where our HQ does not get good AT&T signals (shocker) but we use over 100 iPhones. We purchased an industrial-grade repeater from AT&T and had them install it. We never fall below five bars, it’s solid signal all of the time now. I cannot reproduce the antenna issue with the new iPhone while I am in the building, but I can when I am home. Both with the 3G and 4 iPhones. I am guessing that Apple has an industrial repeater and never experienced an issue while on campus. When they had the new iPhone out in public for testing, they used a 3G case disguise and thus may never have experienced the antenna issue that way too

http://gizmodo.com/5586256/is-apple-silently-recalling-the-iphone-4-nowIs Apple Silently Recalling the iPhone 4 Now? LoadObento : They gave him a Chinese knockoff as a replacement.
